Registration instructions:
  • Complete the Registration Form.
  • Please print or type all information. 
  • Keep a copy for your records.
  • Payment must accompany this form for registration to be processed.
  • Check the registration fee for your appropriate membership category.
  • Early Registration Deadline: September 01, 2009
  • Advance Registration Deadline: October 19, 2009
  • In order to ensure that your meeting materials will be ready for you when you arrive in Cairo, your completed registration form and full payment must be received no later than October 19, 2009. If you cannot meet this deadline, plan to register on-site.
  • IMPORTANT NOTICE! * “Student” applies to undergraduate, graduate and medical students, residents and post-doctoral research trainees. “Paramedical” applies to support personnel working in a routine environment such as nurses and laboratory technicians. Registrations for student and paramedical must be accompanied by a letter from the Head of the Department to prove their status. REGISTRATIONS WITHOUT ACCOMPANYING LETTER WILL NOT BE ACCEPTED AND SUBSEQUENTLY LOW FEES WILL NOT BE VALIDATED. WHEN LETTERS CERTIFYING STUDENT OR PARAMEDICAL STATUS ARE SENT AFTER THE 01 SEPTEMBER DEADLINE, LATE FEES WILL BE APPLICABLE EVEN WHEN REGISTRATION FORMS ARE RECEIVED BEFORE THE DEADLINE.

     

 
     
 
 
Registration Methods:

 

Please note that individual registrations can be made on-line (soon). Alternatively, the registration forms can be used for individual registrations to the annual meeting, and the pre-congress courses. Separate forms should be used for hotel reservations and the optional tours program. Each registration form is for one participant only and his/her accompanying persons. The registration fee for the annual meeting includes :

- admission to all scientific sessions and the exhibition

- admission to the lunch and coffee breaks area

- the final program and abstract book

- a certificate of attendance

- admission to the Opening and Closing Ceremony

 

The registration fee for the pre-congress courses includes:

- admission to one specific course as selected

The pre-congress course fees don't include participation in the main program. 

 

Registrations can only be validated and confirmed when accompanied by payment. Early rates (i.e. before 01 September 2009) only apply when registration AND payment are received BEFORE 01 September 2009. When registrations AND payments are received AFTER 01 September 2009 or AFTER 20 October 2009, then the corresponding late fees will automatically apply.

 

Accompanying Persons

Accompanying person registration fees include:

- admission to the congress premises and exhibit (but not to the scientific sessions)

- lunch and coffee breaks

- admission to the Opening Ceremony

 

 

On-site registrations

On-site registration will be possible. Registration fees valid after 20 October 2009 will apply for on-site registrations.

All Charges due on Bank Transfers have to be paid by the sender.
   
2. Registration fees may be paid by credit card (American Express, MasterCard or Visa) by completing the charge authorization section on the registration form. Please clearly state name of cardholder (exactly as it appears on the credit card), credit card number, expiry date, billing address, authorization signature and date. The registration form should be signed. Card payments with any of these details missing will be considered invalid.
Personal checks cannot be accepted. Remittance should be free of charge to the recipient.
 

                                                                                                                                                    
 
Confirmation:

 

 

Upon receipt of the registration forms with the appropriate fees, confirmation will be sent not later than one month prior to the meeting. Letters of confirmation will be mailed in advance of the meeting only to those participants registered by 04 October 2009.

 
 
 
Cancellation/Refund Policy:

 

A full refund (less 100 Euro administration fee) will be granted to participants unable to attend, provided written notice is received at the Central Office on or before 01 September 2009. MEFS regrets that it will be unable to consider requests received after this date. The date of the receipt of the written request for cancellation by the MEFS Central Office will be considered as the date of cancellation. Group registrations

Companies or institutes are invited to submit group registrations, provided they have a minimum of 5 people to register.

All group registrations should be submitted by means of the official group registration form that should include the name and address details of all participants and the activities for which they have to be registered. Registrations with the company address will be refused unless it is specified that the company employees the participant. Full invoice address, contact person (with direct phone, fax and e-mail address) must also be submitted.

Reservations

In order to secure group registrations at the early (low) registration fees, pre-book the expected number of participants (without names or address details) that may be registered for the main program and/or any pre-congress course before the early fee deadline of 01 September 2009.

This number of pre-booked participants will then be blocked until further details (i.e. name and address details of the participants) are supplied after the early deadline (01 September 2009), but NOT LATER THAN 01 OCTOBER 2009.

After the 01 SEPTEMBER 2009 deadline, late registration fees will apply.

After the 20 OCTOBER 2009 deadline, on-site registration fees will apply.

 

 

 

 

 

 

 

 

 

 

 

 

Payment

After receipt of your reservation they will be processed and confirmed. The confirmation will show a total that will be invoiced unless additional registrations are submitted for the existing group or other changes are made influencing the price. Invoices are payable upon receipt. Payment can be made by bank transfer, or by credit card. Cheques or bank drafts will not be accepted. For bank transfer details, please check the registration form. For credit cards, write the credit card number, expiry date and card owner’s name on a copy of the invoice and have it signed by the card owner. This can then be faxed to the MEFS Central Office for processing.

IMPORTANT NOTICE! - Invoices are payable upon receipt. Registrations without payment will not be validated or confirmed and delegates will not be admitted access to the congress. Payment for registrations and changes received after 10 October 2009 will be processed after the meeting. A final invoice will then be sent.

Replacement registrations can be submitted without penalty but will be charged at the rate applicable at the time of submission.

 

 

 

 

 

 

 

 

 

Cancellations and deadlines

Rules of cancellation for individual registrations also apply to group registrations. Cancellations submitted after 01 September 2009 will be processed but will not be refunded. Before that date, an administrative fee of 100 Euro will be charged for each cancellation. Replacement registrations can be submitted without penalty but will be charged at the rate applicable at the time of submission and before end of October.
